NURSE CARE COORDINATION
Every client will get a registered nurse care manager. The benefit of this service includes the following:
- Develop a personalized care plan to meet the client's needs
- Completes an initial nursing assessment and visits every 45 days and as needed
- Coordinates referrals to other services as needed
- Performs home safety assessments
- Provides training, supervision, guidance, and support to caregivers
- Provides client and family education in care and management of chronic medical conditions
- Coordinates care with physicians, family and other healthcare providers involved in the care plan
- Reduces hospitalizations and emergency room visits
